Designing the exception queue: where automation meets people

No automation handles everything. The exception queue decides whether staff trust it. What to put in front of them and how.

The cases an automation can't handle go to people. If those cases arrive without context, in no order, with no way to feed back, the team will resent the system and work around it.

A good exception queue

  • Reason code — why this item was escalated.
  • Context attached — source documents, what the system already did and its proposed action.
  • Priority and age — oldest and most urgent first, with visible deadlines.
  • One-click resolution paths for the common outcomes.
  • Feedback capture — the resolution is recorded so patterns can be automated later.

Review the top reasons for exceptions monthly. Each frequent reason is either a rule to add or a data problem to fix upstream.
